  • Community Liaison Specialists strive to build positive working relationships with hospital and nursing facility social workers throughout the state by providing outreach, education and training about the Take Me Home Transition Program, including the TMH referral process, eligibility requirements and transition services and support.

  • Transition Coordinators work directly with participants, their families, and their support teams to create a personalized Transition Plan. This plan includes specific services and resources designed to help each individual reach their goals and become an active member of their community.

  • Housing Specialists work closely with TMH participants, their families and support teams, along with their Transition Coordinators to identify and obtain affordable, accessible housing opportunities.
